Child & Whānau Support - Tautoko Hauora Service
Empowering tamariki and whānau to access primary healthcare and social services in Waitaha Canterbury.
This service aims to support the social, behavioural, developmental needs, and wellbeing of tamariki aged 0-14 years.
We work alongside your general practitioner (GP) and offer up to three home visits or phone sessions to:
- Understand and assess the needs of your tamariki and whānau
- Provide short term interventions
- Collect information for your GP for ongoing referrals
- Link whānau and caregivers to services and agencies for ongoing support.
This service is for those who have concerns about the health, behaviour and wellbeing of their tamariki.
Eligibility
- Whānau who are Māori, Pacific, Culturally and Linguistically Diverse (CALD), Youth, Rainbow, low income, and those with disabilities (all ages) where there are tamariki aged 0-14.
- People who are currently enrolled with a Pegasus Practice.
Talk with your doctor or nurse about a referral to this service.
